DISCLOSURE/AGREEMENT/CONSENT
I/We affirm that all the information on this application is true, accurate, complete and correct and agree that if this is not so, my/our application may be denied and/or my/our lease will be held in default and I/we may be subject to eviction. I/we understand that an investigative background inquiry is to be done, including but not limited to identity and prior address(es) verification, criminal history, credit history, employment verification, reason(s) for termination,  work and other references. I/we understand that for the purpose of this inquiry, various sources will be contacted to provide information, including but not limited to various federal, state, municipal, corporate and private sources which may maintain records concerning my past activities relating to possible criminal conduct civil court litigation, driving history and credit performance as well as other information.  I/we authorize without reservation, any company, agency, party or other source contacted to furnish the above information.
I/We affirm and permit that all the information on this application and results of the investigative background inquiry will be provided to the owner/landlord of the property I/we are applying for and their agent, real estate agent, and/or property manager.
